Why Provide a CSR with Your Port Request
A Customer Service Record (CSR) is a document from your current provider that proves you own the phone number you are porting.

What a CSR does
- Verifies your name, address, and account number match carrier records
- Confirms the numbers listed on your port request are active on your account
- Provides the exact details the receiving carrier needs to process the transfer
Why it matters
Submitting a CSR alongside your Letter of Authorization (LOA) significantly improves approval likelihood and prevents delays. Without a CSR, mismatches between your LOA and carrier records are the most common cause of porting rejections.
How to get a CSR
Contact your current phone provider and request a Customer Service Record. Most carriers provide this within 1–2 business days.
Note: Toll-free carriers typically do not provide CSRs. In that case, enter your own account information directly on the LOA along with your account number.
